“The patients make regular follow up visits to a doctor, who completes feedback forms either electronically or on paper. The forms are sent from the doctor to CTG and then scanned with TeleForm to verify for errors. If an error is found the system can convert the form into an electronic form and email it back to the doctor for correction. Corrections only take a few seconds, adjusting only the incorrect data and then the completed forms are re-submitted to TeleForm.
